Evolution Care Solutions is a registered NDIS provider.  We are nurse led and run, with a focus on amazing care with a small group of like minded nurses and support workers.   We can't wait to work with you to help achieve your goals.  

Flexible NDIS community nursing care  delivered when and where it is needed.  Our highly experienced nurses are experts in wound care and can provide a person-centred approach to assist in the management of chronic wounds, diabetes monitoring and support, medication management, catheter care and stoma care. 

Do you need help with personal care?  Or tasks around the home such as cleaning and meal prep?  We can help support participants in maintaining their home environment, providing assistance and development of skills for self care activities.  

We can assist with transport to appointments, social activities and community events. 

Needing help learning life skills?  Our friendly staff can assist in building confidence and social skills, physical skills, important life skills  and improving relationships.  Do you want to get out there and participate in community events?  Let us help you get there.  

Crafts, walks along the river or bay, bush walks and mountain hikes.  Stay tuned for regular events to join.  If you've got a suggestion for an activity please let us know. Our staff love getting outdoors so ask about regular outdoor connection groups.
